About The Position

We are recruiting for one Rheumatology-Immunotherapy Clinical Specialist Pharmacist to cover the Rheumatology Clinic with specialty coverage for Immunotherapy in Scottsdale, Arizona. This is an in-person position with hours from 0800-1700, Monday through Friday. The role involves providing expert consultation and guidance for appropriate Rheumatology/Immunotherapy-affiliated medication use. In collaboration with Rheumatology/Immunotherapy providers, the specialist will offer complete patient care through consultative services in the Ambulatory Care area, support inpatient transitions of care, and actively participate in multidisciplinary patient care planning meetings. The primary goal is to optimize patient outcomes by implementing evidence-based, patient-centered drug therapies, monitoring for drug-related problems, and managing medication costs. The position also supports regulatory requirements, quality programs for safe medication use, and medication stewardship and surveillance systems. Pharmacokinetic consultations, detection and reporting of adverse drug reactions and medication errors, and monitoring drug therapy for appropriateness, dose, regimen, route, duplication, and interactions are key responsibilities. Communication of patient information and treatment efficacy/safety to care providers is essential. The role includes providing in-service education to the Rheumatology/Immunotherapy team and staff, as well as patient and caregiver education on medication therapy and compliance. Leadership and participation in interdisciplinary committees, protocol development, and education of healthcare professionals are expected. Coordination of patient issues and therapy recommendations with pharmacy staff, and support for research activities are also part of the role. Active participation in specialty team meetings, including transitional care/discharge management, scientific research, and educational conferences, is required. Staying updated on changing protocols and clinical pharmacy standards relevant to Rheumatology/Immunotherapy is crucial. Support is primarily for the Rheumatology/Immunotherapy service, including inpatient transition of care needs.

Responsibilities

Provides an expert level of consultation, guidance, and direction for appropriate Rheumatology/Immunotherapy -affiliated medication use.
In conjunction with a team of Rheumatology/Immunotherapy providers, provides complete care to the patient through consultative services in the Ambulatory Care area, with support for inpatient transitions of care, and active participation in multidisciplinary patient care planning meetings.
Optimizes the outcomes of Rheumatology/Immunotherapy patients by providing evidence-based, patient-centered drug therapies and monitors for drug-related problems and medication cost containment.
Supports regulatory requirements and local quality programs related to safe medication use and for systems to promote medication stewardship and surveillance.
Provides pharmacokinetic consultations as required.
Detects, monitors, documents, and reports adverse drug reactions and medications errors.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Communicates ongoing patient information and efficacy and safety of treatment regimen(s) to all care providers.
Provides in-service education to the Rheumatology/Immunotherapy team and staff, performs patient and caregiver education, and counsels patients regarding medication therapy and compliance.
Leads and participates in interdisciplinary committees, protocol development, and education of pharmacists, nurses, physicians, and other providers.
Coordinates current patient issues and therapy recommendations with pharmacy staff.
Supports research activities of the respective specialty team as requested.
Participates actively in all respective area of specialty team meetings, which include transitional care/discharge management, scientific research, and educational teaching conferences.
Keeps abreast of changing protocols and clinical pharmacy standards as they pertain to applicable area of specialty (e.g., Rheumatology/Immunotherapy).
Provides support primarily for the abdominal Rheumatology/Immunotherapy service, including inpatient transition of care needs.
