We are recruiting for one Rheumatology-Immunotherapy Clinical Specialist Pharmacist to cover the Rheumatology Clinic with specialty coverage for Immunotherapy in Scottsdale, Arizona. This is an in-person role with hours from 0800-1700, Monday through Friday. The position involves providing expert consultation and guidance for appropriate Rheumatology/Immunotherapy medication use. In collaboration with providers, the specialist will offer complete patient care through consultative services in the Ambulatory Care area, support inpatient transitions of care, and actively participate in multidisciplinary patient care planning meetings. The role focuses on optimizing patient outcomes through evidence-based, patient-centered drug therapies, monitoring for drug-related problems, and managing medication costs. It also supports regulatory requirements, quality programs for safe medication use, and medication stewardship and surveillance systems. Pharmacokinetic consultations, adverse drug reaction and medication error detection, monitoring, documentation, and reporting are key responsibilities. The specialist will monitor drug therapy for appropriateness, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions, communicating findings to care providers. Patient and caregiver education, as well as counseling on medication therapy and compliance, will be provided. The role includes leading and participating in interdisciplinary committees, protocol development, and educating healthcare professionals. Coordination of patient issues and therapy recommendations with pharmacy staff, and support for research activities are also expected. Active participation in specialty team meetings, including transitional care/discharge management, scientific research, and educational conferences, is required. The specialist must stay current with changing protocols and clinical pharmacy standards relevant to Rheumatology/Immunotherapy. Support for the abdominal Rheumatology/Immunotherapy service, including inpatient transition of care needs, is also part of the role.
