Clinical Pharmacist Specialist
About The Position
The Clinical Pharmacist Specialist ensures safe, appropriate, and cost-effective medication therapies for patients by adhering to evidence-based medicine and established policies, procedures, and protocols. This role involves daily rounding on patients, reviewing electronic medical records, monitoring drug therapy regimens, interpreting medication orders, assisting with pharmacokinetic consults, performing antimicrobial stewardship activities, detecting and reporting adverse drug reactions, documenting clinical activities, participating in quality improvement initiatives, and contributing to educational programs. The specialist also performs medication reconciliations, sustains the formulary, and prepares/dispenses medication orders, including sterile and hazardous products. Additionally, the role includes supervising pharmacy support personnel, maintaining pharmacy areas, and potentially performing duties of the Pharmacy Manager in their absence. Competence is maintained through ongoing licensure and attendance at meetings.
Requirements
- Graduate of an ACPE-accredited School of Pharmacy with a PharmD degree.
- Currently licensed to practice pharmacy in the state of Kansas.
- Ability to work independently with minimal direct supervision.
- Ability to work cooperatively with health care team and pharmacy staff.
- Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
- Ability to set priorities, make critical decisions and respond quickly to emergency requests.
- Ability to exercise sound professional judgment.
- Ability to communicate effectively (orally and in writing).
- Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, chemically dependent, rehabilitation, and skilled-nursing patients.
- No criminal record involving drugs or theft.
- ACLS certified or complete ACLS certification by the end of the probationary period.
Nice To Haves
- Completion of PGY-1 hospital residency preferred.
- Previous experience will be considered (for PharmD waiver).
- At least 3 years of hospital experience as a clinical pharmacist preferred for applicants who did not complete a residency.
Responsibilities
- Rounds daily on all Med Surg patients and other patients as appropriate, providing drug information to patients, families, and healthcare providers.
- Reviews electronic medical records daily, evaluates drug therapy, monitors for contraindications, interactions, allergies, and appropriateness of drug and dose, and contacts prescribers for necessary changes.
- Interprets medication orders (verbal, written, or computer) and transcribes to computerized patient medication profiles accurately or verifies electronic orders.
- Assists with pharmacokinetics consults and drug dosing per organizational protocol.
- Performs antimicrobial stewardship activities by reviewing and interpreting culture and susceptibility data for antibiotic appropriateness and recommends changes.
- Detects and reports suspected adverse drug reactions accurately and in a timely manner.
- Documents all clinical activities and interventions accurately and completely.
- Participates in quality improvement and medication use review activities, collecting data, conducting quality monitors, and maintaining logs.
- Participates in the Palliative Care team and other collaborative groups as assigned.
- Participates in the development and presentation of orientation, education, and training programs, and works with students.
- Performs medication reconciliations on admission and/or discharge, verifies home medications, checks student/intern/pharmacy technician entry, and contacts retail pharmacies for discharge medications.
- Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
- Compounds and dispenses pharmaceuticals, including sterile, hazardous, and parenteral nutrition products accurately.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Supervises and directs pharmacy support personnel, verifies their daily activities, and participates in performance appraisals.
- Works independently with minimal supervision, organizes and prioritizes work assignments, and ensures timely provision of pharmacy services.
- Completes and documents inspections of medication storage areas, identifies and replaces outdated drugs.
- Answers the telephone within 3 rings, identifies self and department, and directs calls appropriately.
- Keeps pharmacy areas and equipment clean, neat, and well organized.
- Performs essential duties of Pharmacy Manager in their absence.
- Follows USP 797 and USP 800 regulations for sterile compounding and handling of hazardous drugs.
- Maintains current pharmacist licensure.
- Attends pharmacy staff meetings and other meetings as assigned.
