About The Position

The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking a Clinical Social Worker for Connecticut Valley Hospital (CVH) in Middletown. CVH is a state-operated psychiatric hospital providing comprehensive inpatient behavioral health services to individuals with complex mental health needs. The hospital brings together an interdisciplinary team of clinical, medical, nursing, rehabilitation, and support professionals who work collaboratively to provide individualized treatment and prepare patients for the next step in their recovery. Requirements

  • Licensure as a clinical social worker in the State of Connecticut.
  • Must possess and retain a license to practice clinical social work pursuant to Sec. 20-195 (m-z) of the Connecticut General Statutes.
  • Must possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. § 1320a-7b(f).

Nice To Haves

  • Experience working in an inpatient or hospital setting treating patients with co-occurring mental health and substance use disorders.
  • Experience utilizing evidence-based practices, such as Integrated Dual Disorder Treatment (IDDT), Dialectical Behavior Therapy (DBT), or Trauma Recovery and Empowerment Model (TREM) in an inpatient setting.
  • Experience working with adults with severe and persistent mental health disorders, substance use disorders, and trauma in an inpatient setting.
  • Experience incorporating cultural sensitivity into client care in an inpatient setting.
  • Experience leading group sessions in an inpatient setting.
  • Experience developing treatment plans for clients in an inpatient setting.
  • Knowledge of sign language and communication ability with the deaf and hearing impaired in certain designated positions.
  • Ability in Spanish oral and written communication in certain designated positions.

Responsibilities

  • Provide discharge planning and clinical social work services to patients on an acute rehabilitation unit with a census of approximately 30 patients, including individuals with significant behavioral health needs.
  • Work closely with an interdisciplinary treatment team to complete psychosocial assessments, provide individual and group counseling, crisis intervention, triage, case management, supportive counseling, and treatment planning.
  • Help patients identify and build upon their strengths, address barriers to recovery, connect with community-based resources, and work toward greater social and emotional stability.
  • Develop, implement, and evaluate individualized treatment and rehabilitation plans.
  • Coordinate effective discharge plans so patients can transition to the most appropriate level of care.
  • Collaborate with community providers and other behavioral health professionals.
  • Document clinical services.
  • Monitor treatment progress.
  • Use clinical judgment to help patients move toward successful community reintegration.
  • Serve as a case manager and advocate for clients that have received or need to receive behavioral health care services.
  • Manage a caseload of consumer behavioral healthcare insurance complaints concerning coverage and/or payment.
  • Review and assess medical records including psychological assessment and treatment plans, concerning but not limited to mental, emotional, behavioral and addictive disorders to determine appropriate treatment plans and eligibility for insurance benefit payment.
  • Participate in insurance appeal hearings and serve as clinical expert to defend and demonstrate that the diagnostic assessment and treatment plans are appropriate and required to address client behavioral healthcare needs.
  • Provide clinical supervision to Licensed Master Social Workers.
  • Consult and collaborate with other behavioral health providers to improve delivery of services to clients.
  • Provide forensic evaluations and court testimony as required.
  • Maintain social work treatment and standards as required.
  • May participate in research projects.
