About The Position

The State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking a qualified and experienced Advanced Clinical Social Worker to join the Western Connecticut Mental Health Network (WCMHN) Mobile Crisis Team in Torrington, CT. This part-time position (20 hours) operates on the 2nd Shift (3:30pm - 12:00am), with a rotating schedule of Week 1: Friday & Saturday and Week 2: Monday, Tuesday & Wednesday. The WCMHN's mission is to provide a comprehensive and effective recovery-oriented system of mental health and addiction services. The Access Center provides Mobile Crisis Services to a specific service area and coordinates care with local hospitals. The role involves functioning as a member of an interdisciplinary Mobile Crisis Team, providing crisis evaluations and interventions in the community. This includes assessing risk, formulating interventions, administering Emergency Certificates, and providing services via a Regional Hotline. The position also involves intake services, completing psychosocial and risk assessments, and coordinating care with various agencies, providers, and law enforcement. Responsibilities include linking community members to care, managing risk, providing coverage for the Jail Diversion program, completing assessments for the court, coordinating treatment participation, providing individual and group psychotherapy, developing recovery plans, participating in case conferences, documenting interventions in the electronic health record, and participating in committees and trainings. Additionally, the role may involve shift oversight and clinical supervision of staff, students, and interns.

Requirements

  • One (1) year of experience as a licensed clinical social worker.
  • Possess and retain a license to practice clinical social work pursuant to Sec. 20-195 (m-z) of the Connecticut General Statutes.
  • Possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. § 1320a-7b(f).

Nice To Haves

  • Experience performing intake (psychosocial) assessments and initial care planning.
  • Experience completing community-based crisis assessments.
  • Experience completing risk assessments and developing risk mitigation plans.
  • Experience providing trauma informed services to individuals with severe and prolonged mental illness, addiction and co-morbid medical conditions.
  • Knowledge of sign language and communication ability with the deaf and hearing impaired in designated positions.
  • Ability in Spanish oral and written communication in designated positions.
  • Ability to travel.

Responsibilities

  • Functions as a member of an interdisciplinary Mobile Crisis Team that provides crisis evaluations and interventions in the community.
  • Clinically assess risk in the community, formulate immediate interventions, and administer Emergency Certificates, as clinically indicated.
  • Provides services via the Regional Hotline with the skills to access/triage/activate resources for people who request information, support or crisis intervention.
  • Provides Intake services by completing a psychosocial assessment, risk assessment and other admission paperwork for the purpose of admitting clients to the agency.
  • Communicates and works collaboratively with contract agencies, community providers, State of CT DMHAS and community hospitals as well as local and State police.
  • Links community members to appropriate care, provides community resource information and manages risk for negative outcomes in the community.
  • Provides coverage for the Jail Diversion program as assigned by the program supervisor.
  • Completes assessments and makes clinical recommendations for treatment as requested by the court.
  • Coordinates care and monitors participation with psychiatric and substance abuse treatment services.
  • Provides individual and group psychotherapy to a small caseload.
  • Develops and implements person-centered recovery plans to guide treatment.
  • Participates in case conferences and critical incident reviews.
  • Documents treatment interventions in the electronic health record and completes reports and screenings in accordance with agency policies and procedures.
  • Participates in organizational committees and trainings, as assigned.
  • Provides shift oversight for the Access Center staff as assigned by the program supervisor.
  • Provides clinical supervision to assigned Access Center staff and to students and/or interns as assigned by the program supervisor.
