About The Position

The Clinical Social Worker collaborates with patients, families, and healthcare professionals to evaluate patients’ medical and psychosocial needs and assess client needs as part of an interdisciplinary team. This position works collaboratively with healthcare team members to develop and implement individualized treatment plans that support patient-centered care. The Clinical Social Worker provides services across the Emergency Department (ED), Inpatient, and Palliative care settings, helping patients and families navigate complex medical, emotional, and social needs.

Requirements

Master's Degree Social Work required or Master's Degree Mental Health & Behavioral Medicine required
NH Clinical Social Worker License Required

Strong written and verbal communication skills.

Responsibilities
