About The Position

Under the supervision of the Clinical Supervisor for the Citywide Supportive Housing Program, the Clinical Social Work I/II (CSW I/II) works with San Franciscans living in Permanent Supportive Housing who struggle with issues of mental and physical health, substance use issues and housing retention to collaborate with community providers and link residents to appropriate outpatient services and provide housing stabilization to prevent homelessness. The CSW I/II will carry a caseload of residents from one of the DISH supportive housing sites and provide individual treatment including case management services, psychotherapy, crisis intervention and 5150 assessments, substance use treatment, and psycho education, group treatment, community events; intake, assessment, service planning and consultation and other duties as assigned. The CSW I/II maintains client records according to California, City and County requirements; and participates in an interdisciplinary treatment team. CSW I/II attends weekly staff meetings and participates in all required training sessions. Plus, other duties as assigned. CSW I/II demonstrates a commitment to anti-racism, social justice and the ability to engage and respect a diverse community of clients and peers. The CSW I/II will coordinate closely with property management staff from DISH, DPH Nursing and all other service providers to develop treatment strategies; advocacy and coordination with other agencies, families, and clients; track client contacts in Epic and One Systems, complete initial and annual assessments and service plans, referrals, and linkage; coordinate closely with hospitals and clinic medical providers and the candidate will also perform other duties as assigned. Clients typically require routine assistance, individualized treatment approach and support from a community of providers. Primary emphasis on treatment uses a client centered approach to prepare individuals with disabilities in attaining optimal function, including psychological, social, and vocational function in the context of their personal goals, abilities, perception of quality of life and maximize housing retention with the use of a combination of clinical case management and advocacy. The incumbent must foster and embrace the UCSF PRIDE The job will involve the ability to lift 50 lbs, walking and climbing stairs and performing outreach to clients in their homes daily. DEPARTMENT DESCRIPTION: DEPARTMENT OF PSYCHIATRY ZSFG ZSFG is dedicated to providing services that integrates medical, psychiatric, and addiction treatment. ZSFG is the largest provider of acute psychiatric care and the only San Francisco provider of psychiatric emergency care.

Requirements

• Masters degree in social work, psychology or related field from an accredited university and registered with the Board of Behavioral Science as An Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT).
• Masters degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science.
• Ability to work with clients and program staff from diverse cultural and social class backgrounds
• Understanding of mental health issues and sensitivity toward mental health clients
• Ability to work cooperatively and communicate effectively with others.
• Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
• Ability to follow oral and written instructions accurately, and be detailed oriented.
• Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes.
• Knowledge of medical and/or psychiatric terminology

Nice To Haves

• Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
• One year experience doing community based case management and the ability to do outreach including to SRO hotels and scattered site housing locations.

Responsibilities

• Provides ongoing mental health services, housing navigation and stabilization services, eviction prevention services, psychotherapy, advocacy, Crisis Intervention, Case Management and Outreach to a caseload of severely mentally ill clients living in scattered site housing throughout SF. Makes case terminations, treatment plans, and discharge plans, when clinically appropriate.
• Develops a comprehensive individual service plan for each client, integrating knowledge of a wide range of human service resources. Provides consultation to a multi-disciplinary treatment team on the psychosocial ramifications of psychiatric illness. Facilitates treatment coordination among various agencies and advocates for and links clients to needed services. Provides consultation to and maintains liaison with campus and community programs, including educational, legal, and social organizations involved in case management. Acts as liaison to inpatient units to facilitate referrals and service treatment.
• Screens referrals for meeting program criteria; conducts intake of new clients, including assessment, community functioning, and service needs, when appropriate. This entails assessment of presenting problems, psychosocial history, mental status and diagnosis (DSM-V). Responds to unscheduled crisis and urgent needs, utilizing crisis intervention techniques, verbal de-escalation and providing 5150 services as needed.
• Maintains client’s records according to California, City and County requirements. Records data in client’s charts. Meets program productivity standards. Assists in processing quality improvement requests. Completes billing and other documentation of services in a timely fashion. Attends staff and clinical team meetings, and participates in training sessions (e.g., In services, grand rounds). Identifies program problems and makes recommendations to supervisory and administrative staff.
• Provides education to the community regarding program services, client needs and treatment. Assists in planning and providing in-service to staff. Provides orientation and didactic training to interns. If desired and appropriate, supervises student(s) and participates in student training program meetings.
