About The Position

Under direct supervision, the Clinical Social Worker I conducts psychosocial assessments and interviews with patients at Midtown Health Center to identify behavioral health needs and appropriate referrals and resources. The role connects patients to short- and long-term behavioral health services and supports. The position also provides basic interventions, develops safe and patient-centered care plans, coordinates services, and supports patients and families in navigating the healthcare system.

Requirements

Bachelor’s degree in Social Work (BSW) accredited by the Council on Social Work Education (CSWE).
No experience required.

Nice To Haves

LBSW (Bachelor Social Worker) licensure from the Maryland Board of Social Work Examiners.
Knowledge of behavioral health services offered throughout the Baltimore area.
Experience with therapy.

Responsibilities

Conduct psychosocial assessments and provide basic interventions using social work approaches, including problem-solving, crisis intervention, task-centered, and solution-focused care.
Develop safe, patient-centered care plans.
Connect patients with behavioral health services, community resources, and other supports.
Follow up to monitor progress, confirm linkage to care, and assess whether resources meet patient needs.
Provide immediate and follow-up referrals to promote patient stability and continuity of care.
Consult with colleagues and interdisciplinary team members regarding assessment, care planning, interventions, and coordination of services.
Provide support, education, and advocacy to patients and families and encourage active participation in care.
Participate in interdisciplinary meetings and help patients navigate the healthcare system and access appropriate services.
Complete timely and accurate documentation in accordance with organizational and regulatory requirements.
Maintain confidentiality and obtain releases of information (ROI) when required.
Perform other duties and responsibilities as assigned by the supervisor.
