About The Position

Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II (CSW I/II) works doing in person outreach in the single room occupancy hotels (SROs) in San Francisco’s Tenderloin, South of Market, and Mission neighborhoods. The permanent supportive housing sites where you will be doing outreach house formerly homeless adults who have been placed in housing through a subsidy from the city. The candidate will seek to engage high risk mentally ill, medically complex and/or substance abusing clients to increase housing retention and lessen the risk of eviction and a return to homelessness through building relationship and linking clients to needed supports. The individual will hold a caseload of clients for whom they will be providing community based clinical case management which includes; engagement, assessment, treatment planning, psychotherapy, crisis intervention, 5150 assessments and holds, substance use treatment, psycho education, referrals to community supports, coordination and collaboration with property management and support services staff at the housing sites, advocacy for services, communication with families, and mandated reporting as needed. They will coordinate and consult closely with acute hospitals settings and outpatient medical providers to increase the continuity of care for their clients. They will track and document their interventions, assessments and referrals in a timely manner using an electronic medical record (Epic). The candidate will also perform other duties as assigned. The CSW I/II should possess a basic understanding of the DSM V mental health diagnostic criteria, Harm Reduction principles, Trauma Informed Care and Anti-Racism. Working with diverse and multicultural teams, clients, and community, the incumbent will participate and contribute to culturally focused clinical interventions and will practice a commitment to cultural humility and anti-racism. The Supportive Housing Teams have a two hour weekly clinical team meeting where we foster a supportive learning and working environment by engaging in clinical trainings, sharing community resources, peer support, and presenting clients to the team for feedback and support. There is also an hour of clinical supervision weekly to each clinician. The job will involve the ability to lift 50 lbs, walking several miles a day and climbing stairs. The Clinical Social Worker I/II (CSW) provides comprehensive psychosocial assessment, crisis intervention and 5150 services, counseling, brief therapeutic interventions, consultation, education, outreach, community linkages, and facilitates the plan of care for patients with a particular focus on high-risk populations. Additionally the incumbent provides mental status examinations and diagnoses using the Diagnostic Statistical Manual (DSM5-TR); consultation to a multi-disciplinary treatment team; acts as liaison to hospital and community resources; maintains client records according to California State Medi-Cal and San Francisco City and County requirements; record data as required by Department of Public Health, attends staff meetings and participates in training sessions and performs other duties as assigned. Requirements

CSW I: Master’s degree in social work, psychology or related field from an accredited university and registered with the Board of Behavioral Science as An Associate Clinical Social Worker (ACSW) or Intern Marriage Family Therapist (IMFT), Associate Professional Clinical Counselor, or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position;
CSW II (in addition to the above): Master’s degree in Social Work, Psychology, Counseling, or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
Ability to work collaboratively and communicate effectively with others.
Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
Ability to follow oral and written instructions accurately, and be detailed oriented.
Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes.
Knowledge of medical and/or psychiatric terminology

A preliminary job offer and assignment may be extended pending successful background clearance.
An understanding of mental health issues, the recovery model, and sensitivity toward mental health clients
Ability to work with clients and program staff from diverse cultural and social class backgrounds
Case Management experience
Experience working with people who have severe mental health symptoms and related behavior, people who use substances, and people experiencing homelessness.
Experience doing community-based case management and the ability to do outreach.

Responsibilities

Provides ongoing mental health services, including psychotherapy (Individual, Group and Family), crisis intervention, clinical case management, and intensive outreach and engagement to a caseload of formerly homeless clients living in permanent supportive housing site or scattered site housing. Help client stabilize, maintain housing and link to longer-term ongoing mental health, substance abuse, medical services and other programs. Make clinically appropriate discharge plans and terminations.
Coordinate services closely with housing providers and other community partners to develop successful treatment strategies. Treatment advocacy and coordination with other agencies, families and clients. Performs assessments, facilitates linkage to ongoing care and provides 5150/Crisis intervention services as needed.
Track client contacts, assessments, referrals and eviction. Utilize Medi-Cal billing for services. Maintains client records according to City and County and Medi-Cal requirements in Epic or other EMRs. Records data in client’s charts. Meet program productivity standards. Assists in processing quality improvement requests. Completes service sheets and other documentation of services in a timely fashion. Attend staff and other assigned meetings, and participates in training sessions (e.g., In-services, grand rounds). Identifies program problems and makes recommendations to supervisory and administrative staff. Other duties as assigned.
