Requirements

• Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment); or Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor’s degree in social work or social welfare; or Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare; or A master's degree in social work or social welfare.
• You must possess an active license as a Social Worker or Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
• You must meet the PA residency requirement.
• You must be able to perform essential job functions.
• A conditional offer of employment will require a medical examination.
• This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

Responsibilities

• Join interdisciplinary team meetings, complete documentation, and integrate psychosocial assessments with person-centered plans
• Provide behavioral assessments, implement appropriate interventions, and offer guidance to staff on behavior management strategies
• Review resident rights, assist with advanced directives, investigate grievances, and verify voting status
• Provide individual and group counseling, offer crisis support, and guide residents and families through emotional challenges
• Organize transfers by addressing needs, coordinating with families, arranging services, and ensuring transportation and follow-up care
• Follow safety rules, support quality improvement efforts, use protective equipment, and maintain strong professional relationships
