About The Position

Concordia Visiting Nurses is seeking a passionate problem-solver with a knack for crafting elegant solutions to join their rapidly growing team as a Clinical Scheduler. This role will involve shaping the future of Concordia Visiting Nurses by managing daily and weekly schedules for clinical staff, ensuring appropriate staffing levels based on client needs, preferences, and availability. The Clinical Scheduler will also be responsible for communicating schedule updates and changes to clients, families, and clinical staff, responding proactively to last-minute changes and urgent client needs, maintaining accurate records, and collaborating with various teams to ensure smooth transitions of care. A strong focus on customer service, professionalism, empathy, and patient-centered interactions is essential.

Requirements

  • High school diploma or GED equivalent.
  • Personable & Empathetic: A friendly and positive demeanor with exceptional interpersonal skills, fostering trust and rapport with clients and caregivers.
  • Highly Organized: Excellent organizational skills with meticulous attention to detail to manage complex schedules and multiple priorities effectively.
  • Strong Communication: Clear and effective verbal and written communication skills for interacting with diverse individuals.
  • Problem-Solver & Proactive: Ability to anticipate potential scheduling conflicts and find innovative solutions to ensure seamless care delivery.
  • Technical Proficiency: Comfortable using scheduling software and Electronic Health Records (EHR) systems.

Nice To Haves

  • Knowledge of medical terminology and home health care operations is a plus.

Responsibilities

  • Creating and managing daily and weekly schedules for clinical staff, ensuring appropriate staffing levels based on client needs, preferences, and availability.
  • Effectively communicating schedule updates, changes, and important information to clients, families, and clinical staff.
  • Responding proactively to last-minute changes, call-offs, and urgent client needs, finding efficient solutions to maintain continuity of care.
  • Maintaining accurate records of schedules, authorizations, and client information within the scheduling software and electronic health records (EHR) systems, adhering to all HIPAA regulations and compliance standards.
  • Working closely with the Clinical Manager, Intake team, and other staff members to coordinate admissions, initial visits, and ensure smooth transitions of care.
  • Demonstrating a strong focus on customer service, ensuring all interactions are professional, empathetic, and patient-centered.
