About The Position

The position of Clinical Risk and Safety Manager at Intermountain Health will lead local Community Clinics, Hospitals, Specialties, and Homecare/Hospice for clinical risk management and patient safety services. This includes identifying and responding to clinical concerns resulting in or potentially resulting in patient harm. Facilitate early investigation of concerns, and support process improvement specific to Serious Safety Events, Reportable Events, and Sentinel Events. Provides support for the development of a culture of high reliability and the measurement of care, identifying opportunities and executing on strategies for performance improvement in the following areas: Outcomes - highly reliable processes, delivering exceptional outcomes and / or optimal health Safety - Zero Harm to patients and caregivers driven through a culture of high reliability, continuous improvement and leadership Patient Experience - relationship-centered care grounded in empathy, dignity, respect and collaboration Clinical Experience is a resource to the system and drive organizational performance that is built on relationships, a shared vision of excellence in and environment of achievement and local accountability.

Requirements

• Bachelor's degree required.
• Education must be obtained through an accredited institution and will be verified.
• 3 years Clinical Experience
• Customer/Patient Relations experience
• Strong written and verbal communication skills with an ability to teach and train peers and colleagues.
• Experience and demonstrated competence in solving problems and liaising between multiple stakeholders.
• Proficient in use of software such as Word and Excel in order to generate documents and data reports.

Nice To Haves

• Clinical risk and Safety management experience is highly preferred
• Experience working in a complex health system (hospitals, ambulatory clinics, post-acute care, etc.)
• Experience in formal business letter composition and report writing.
• Experience with quality improvement and data analysis.
• Experience utilizing Microsoft Excel
• Experience utilizing AI Tools.
• Clinical Risk Management experience with successful progression

Responsibilities

• Support a proactive, comprehensive strategic patient safety agenda for the system that inspires caregiver engagement, inter-professional collaboration, the identification and use of evidenced based practice and management among all facilities in the system while differentiating Intermountain Health as a high reliability organization nationally.
• Serve as a consultant and educator to assist local clinical and administrative leaders in keeping the patient safety agenda at the forefront.
• Responsible to consult with and assist caregivers in identifying patient safety issues and risks exposures associated with new, redefined, expanded or established clinical processes and services.
• Responsible for the investigation and assisting in the identification of appropriate interventions.
• Facilitate appropriate hand-off to operational leaders to monitor implement and monitor resolutions and their sustainability.
• Perform investigation needed to support early intervention and potential disclosure. This would include a review of clinical facts, preparing a high-level timeline, and understanding what the patient knows about the event.
• Facilitate hand-off to Quality/Patient Safety and/or to Legal/Claims as appropriate.
• Determine whether event qualifies as a Serious Safety Event, Reportable Event, or Sentinel Event. Follow reporting procedures accordingly.
• Initiate and lead the appropriate analysis process, which might include Special Cause, Common Cause or Root Cause Analysis.
• Prepare information and documentation required for the Serious Event Review Panel (SERP) process.
• Support the process of early communication and/or disclosure to patients involved with clinical errors.
• Provide documentation needed to the principal discloser, determine with key stakeholders what should be included.
• Work with Claims and Legal to determining what (if any) compensation should be offered to the patient in the disclosure setting.
• Coordinate with central CRM leaders to prepare and communicate clinical risk management information, reports and analytics to leaders and stakeholders.
• Assists in the development of Risk Management policies and procedures specific to appropriate areas of risk concern, correlating them with other policies and programs associated with Intermountain's clinical and service quality efforts.
• Coordinates the review, investigation, documentation and follow-up on patient/family complaints received directly not otherwise handled by Clinical Relations in a timely and responsive manner.
• This includes events received via handoff from Patient Experience.
• Works with Patient Account Services on event-related billing holds and adjustments.
• Provide clinical risk management education, and training or consultation, as appropriate to department managers, administration, and all other care givers as needed and requested.
• Provides risk management consultations under the direction of Legal as needed to all caregivers and requested.
• Collaborates on clinical process improvement needed as identified from event and claim management.
