About The Position

The risk and patient safety specialist reports directly to the Manager of Quality and Risk, working alongside a larger team to advance safety, reduce harm and improve patient outcomes. The risk and patient safety specialist will have two key areas of focus: Advancing Patient Safety and Reducing Harm through risk mitigation. Patient Safety within this role requires the incumbent to partner in the planning, development, implementation and evaluation of patient safety activities, including the development of an organizational patient safety plan, advancing a Just Culture at Southlake, implementing harm reduction measures and investigation through standard methodologies (e.g., Root Cause Analysis, FMEA, Hierarchy of Interventional Effectiveness, Common Cause Analysis) to prevent future harm. This role will also require the incumbent to provide education on patient safety incidents, and partner with clinical programs, leaders and physicians to embed patient safety activities within patient care. At the corporate level, the incumbent will leverage their expert knowledge and skills in risk management (e.g., Risk Assessment Checklists, Harm events), accreditation, and quality improvement to reduce harm and assist in better patient outcomes. At the program level, the incumbent will facilitate the integration of accreditation standards into practice, conduct incident reviews, complete root cause analysis and design improved systems for tracking and evaluating areas of vulnerability.

Requirements

  • University degree in a health care discipline or relevant experience
  • 5 years of hospital experience (clinical experience preferred)
  • 3 years’ experience with Patient Safety or Risk Management projects/initiatives
  • 3 years’ experience with incident investigations including RCA, common cause analysis, multi-case reviews
  • Strong analytical skills including leveraging large data sets
  • Accreditation experience in facilitating implementation of ROPs, Priority Processes or standards
  • Strong understanding of quality improvement methods/tools i.e. LEAN, FMEA, RCA
  • Knowledge of healthcare legislation and regulatory environment
  • Excellent analytical, critical thinking, problem solving, negotiation and conflict resolution skills
  • Strong interpersonal, investigative, teaching and facilitation skills
  • Confident, flexible, service orientated and capable of managing multiple, competing priorities.
  • Superior verbal, written, listening and presentation communication skills
  • Ability to influence others and to establish credibility with all levels of the organization, challenging the status quo and being a catalyst for positive change.
  • Networks with other organizations on most effective way to meet standards, mitigate risk and improve quality
  • High level of integrity, trust and creditability
  • Demonstrated ability to work independently and with multidisciplinary teams
  • Proficiency in the Microsoft suite of software, including MS Word, MS Excel, MS Project and PowerPoint.
  • Knowledge of incident management systems (RL Solutions), electronic health record platforms is an asset
  • Demonstrated individual leadership skills

Responsibilities

  • Conducts proactive risk assessments as delegated by Risk Manager to identify quality of care concerns and facilitate implementation of risk management and quality improvement initiatives.
  • Participates in Root Cause Analysis with programs/departments as required for patient safety incidents identified through the incident reporting system or other quality review processes.
  • Implementing and leading Morbidity and Mortality Rounds within programs
  • Reviews and monitors implementation of patient safety/risk management policies.
  • Promotes patient safety requirements in alignment with Accreditation requirements and patient safety organizations.
  • Implements key practices to reduce patient harm, maximize patient safety and minimize organizational liability /exposure.
  • Provides support on quality and patient safety processes to assist the front-line staff, leadership and organization committees in the pursuit of risk mitigation, patient safety and quality improvement.
  • Supports and participates in the data collection, preparation, and delivery of reports related to risk management and patient safety.
  • Support leadership with the trending and analysis of specific incidents that carry a potential or actual liability for the organization.
