About The Position

We are seeking a Clinical Quality Specialist to support clinical quality improvement initiatives, population health strategies, and performance improvement efforts across HCN programs and participating health centers. This role will support quality-focused projects and operational initiatives that drive improved patient outcomes, regulatory compliance, and workflow optimization. The Clinical Quality Specialist will collaborate closely with Clinical Quality, Epic/IT, Healthy Planet, Enterprise Data & Analytics, and health center teams to support quality measure performance, reporting needs, workflow adoption, and organizational priorities. This role will assist with monitoring clinical quality data, identifying performance gaps and trends, supporting action plans, and helping health centers improve workflows and quality outcomes through data-driven recommendations and operational support.

Requirements

Bachelor’s degree in healthcare, public health, nursing, health administration, or related field required; or equivalent combination of education and experience
Experience supporting clinical quality improvement, population health, healthcare operations, or performance improvement initiatives
Strong understanding of clinical quality measures, quality improvement principles, and healthcare performance metrics
Ability to interpret clinical quality data, measure definitions, trends, and performance gaps
Strong organizational, communication, collaboration, and project coordination skills
Ability to translate data and operational priorities into actionable recommendations and workflow improvements
Experience working with dashboards, reporting tools, or healthcare data systems

Nice To Haves

Experience supporting FQHCs, ambulatory care settings, or community health center operations
Knowledge of UDS, HRSA reporting, value-based care, and population health initiatives
Experience with Epic, Healthy Planet, COSMOS, or other healthcare reporting and analytics tools
Experience supporting workflow optimization, end-user adoption, or operational improvement initiatives
Quality improvement certifications such as CPHQ, Lean Six Sigma, PMP, or related certifications preferred
Epic Ambulatory Certification, Clinical Informaticist Certification, or COSMOS Super User Badge preferred (HCN-supported)

Responsibilities

Support clinical quality improvement initiatives across multiple programs, measures, and participating health centers
Assist with implementation and monitoring of quality improvement strategies, workflows, and performance improvement activities
Review clinical quality data to identify trends, gaps, and opportunities for operational and quality improvement
Support health centers with quality measure interpretation, reporting questions, documentation workflows, and performance improvement efforts
Coordinate with internal teams to support Epic quality tools, Healthy Planet workflows, dashboards, registries, and reporting resources
Assist with regulatory and program reporting activities including UDS, HRSA, value-based care, and grant deliverables
Maintain project documentation, timelines, meeting notes, status updates, and follow-up items within Salesforce/Mission Control and related systems
Prepare presentations, summaries, reports, and communication materials for leadership and participating health centers
Support workflow optimization, training reinforcement, and adoption of clinical quality initiatives
Serve as a liaison between health centers and internal teams to support communication, collaboration, and issue resolution
Support additional clinical quality and organizational initiatives as assigned
