About The Position

The Clinical Practice Nurse 1 reports to the Clinical Leader or Clinical Manager and works under the direction of practice providers to deliver high-quality patient care, including assessment, treatment, planning, and evaluation of care. The nurse applies knowledge of growth and development across the lifespan, assesses clinical data reflecting patients’ health status, and interprets information to identify age-specific care needs. The role involves building caring relationships with patients, families, and caregivers; collaborating with the health care team; and using sound judgment when performing nursing interventions.

This is a part-time position for 28 hours per week, scheduled Mondays, Wednesdays, and Thursdays, in Internal Medicine in Concord.

Requirements

• Associate degree or diploma in Nursing from an accredited School of Nursing.
• New Hampshire Board of Nursing license.
• Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers certification.
• Demonstrated ability to interact effectively and positively with multidisciplinary staff, patients, and others.

Responsibilities

• Establish priorities for daily activities and maintain efficient patient flow.
• Triage patient phone calls according to established guidelines and prescribed formats to identify health problems or concerns and provide quality patient care.
• Administer prescribed medications.
• Educate patients in response to identified needs.
• Document health information, interventions, and outcomes in the patient electronic medical record.
• Delegate patient care activities using the five rights: right task, person, direction, circumstance, and supervision; ensure completion and documentation, and provide feedback to the delegatee and manager.
• Apply purposeful, organized thinking based on the nursing process to make sound clinical judgments and decisions.
• Complete practice-specific clinical skill competencies annually.
