Requirements

  • Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or, PharmD in PHARMACY from an accredited School of Pharmacy.
  • Coursework/Training - Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent education/experience.
  • Oregon Pharmacist License upon hire.
  • Certification from Board of Pharmacy Specialties within 2 years of hire.
  • TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.

  • Board of Pharmacy Preceptor license when eligible and where applicable upon hire.
