Requirements
- Bachelor's Degree in Pharmacy from an accredited School of Pharmacy or, PharmD in Pharmacy from an accredited School of Pharmacy.
- Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent educ/experience.
- Oregon Pharmacist License upon hire.
- Certification from Board of Pharmacy Specialties within 2 years of hire.
- TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
