Requirements

  • Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD)
  • Licensed and in good standing with the Ontario College of Pharmacists (Part A)
  • Injection certification required
  • Current CPR certification (for direct patient care roles)
  • Demonstrated strength in patient communication, clinical follow-up, and chronic disease management
  • Proficiency in English is required for this position.

Responsibilities

  • Conduct patient onboarding and medication initiation support, including education on therapy expectations, administration techniques (e.g., injections), side effect management, and adherence strategies.
  • Perform regular clinical follow-ups to assess treatment response, tolerability, adherence, and patient-reported outcomes; escalate or collaborate with prescribers when therapy adjustments are needed.
  • Deliver comprehensive medication reviews and reconciliations, identifying drug therapy problems, optimizing regimens, and ensuring safe, evidence-based care.
  • Provide ongoing patient counselling and disease state education, addressing clinical questions and empowering patients in self-management of chronic conditions.
  • Develop and maintain clinical education materials (e.g., patient guides, injection training resources, disease state information, call scripts) to support consistent, high-quality patient care.
  • Collaborate with physicians, nurses, and other healthcare providers to support coordinated, patient-centered care and resolve therapy-related issues.
  • Monitor and document clinical outcomes and patient interactions, ensuring accurate and compliant records aligned with regulatory and organizational standards.
  • Support adherence programs and persistence initiatives, including proactive outreach and follow-up for high-risk or priority patients.
  • Participate in quality improvement initiatives to enhance patient experience, clinical outcomes, and operational workflows.
  • Assist with training and mentorship of team members in patient counselling, onboarding processes, and clinical best practices.
  • Contribute to program development and special projects, including new patient care pathways, education initiatives, and service enhancements.
