Requirements
- One year of specialized clinical pharmacy experience.
- A current license to practice as a Registered Pharmacist issued by the California State Board of Pharmacy.
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

- Successful completion of a United States pharmacy residency program in clinical pharmacy.
Responsibilities
- Provide specialized clinical pharmacy services in the delivery of pharmacotherapeutics or pharmacokinetics in specialized medical care areas.
- Design, implement and conduct medication usage evaluations.
- Deliver drug information and drug education services.
- Participate in the development and continual monitoring of patient drug therapy treatment plans and patient outcomes in clinical specialty programs.
- Perform patient assessments and monitor the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques.
- Attend daily rounds and medical conferences with physicians in specialty medical areas to evaluate patient drug therapy regimens.
- Interview patients to obtain information regarding drug use, drug allergies and sensitivities, and medication histories; assess compliance and counsel patients on appropriate drug use.
- Participate in ongoing medication usage evaluation programs by assisting in the establishment of criteria for cost effectiveness/control utilization use, process indicators, and monitoring parameters; conduct evaluations to determine compliance to the established medication use evaluation criteria; summarize results and submit recommendations to the Pharmacy Chief or the Pharmacy and Therapeutics Committee and assist in establishing drug monographs.
- Participate in developing critical pathways to ensure efficient medication usage.
- Provide comprehensive drug information services for healthcare providers and patients, including developing pharmaceutical newsletters, patient education materials, and assessment and evaluation of formulary requests to the Pharmacy and Therapeutics Committee.
- Develop and conduct in-service and educational programs regarding drug therapy for healthcare providers, pharmacists, and students on a variety of health related services.
- Critically analyze, evaluate, and prepare recommendations of medical literature in relation to medication therapy.
- Interpret and apply pharmacokinetic drug data in recommendations for patient-specific drug dosage regimen; formulate individualized drug regimens based on patient specific parameters, patient diagnosis, concurrent disease states, pharmacokinetics, pharmacoeconomics, and the patient's condition.
- Advise and assist in implementing cost containment strategies for the usage of pharmaceutical products.
- Review journals and periodicals and meet with pharmaceutical product specialists to discuss current market conditions, managed care implications, disease state management, and obtain information to implement cost containment strategies and develop therapeutic outcome studies.
- Participate in performance improvement activities to ensure optimal pharmaceutical care.
- Dispense medication as necessary.
