About The Position
The Clinical Pharmacist verifies medication orders and provides clinical services for assigned areas. This role serves as a drug information resource for nurses, physicians, patients, and families, and fulfills Staff Pharmacist responsibilities as needed. The Clinical Pharmacist provides patient-centered care and integrated pharmacy services in collaboration with other healthcare professionals, supports medication therapy management programs, and helps develop and evaluate medication-related practices and protocols.
Requirements
- Bachelor of Science degree in Pharmacy.
- Pharmacy practice residency or equivalent hospital experience (3 postgraduate years).
- Current registered pharmacist license in the State of Florida.
- Pharmacy license.
- BLS certification within 30 days of hire; ACLS and/or PALS if code attendance is required.
Nice To Haves
- Doctorate of Pharmacy degree.
- Board Certification in Pharmacotherapy (BCPS) or another specialty corresponding with the practice area.
Responsibilities
- Perform all general responsibilities of a Staff Pharmacist.
- Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
- Attend medical rounds routinely during decentralized shifts and consistently document pharmacist interventions.
- Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
- Assist in developing and implementing facility-specific protocols, guidelines, and reference documents related to medication therapy management.
- Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice based on findings.
- Apply problem-solving, intervention, evaluation, and leadership skills.
