About The Position

The University of Iowa Healthcare Department of Pharmaceutical Care is seeking full-time Clinical Pharmacists for its Specialty Pharmacy and Ambulatory Care team. This role involves providing comprehensive pharmaceutical care to patients within the UI Specialty Pharmacy Program and UI Healthcare Ambulatory Care Pharmacies. Services include clinical support, patient education, dispensing, and monitoring of medications through various channels such as face-to-face, mail order, and telehealth. The pharmacist will act as a role model for staff, residents, and students, focusing on improving professional capabilities and expanding ambulatory pharmacy services. This position requires rotation between an offsite central fill pharmacy and office locations in Iowa City, supporting both adult and pediatric ambulatory care operations at the UI Health Care Medical Campus. Adequate training for Specialty Pharmacy and Ambulatory Care Services will be provided.

Requirements

  • PharmD (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy
  • One (1) year of applicable pharmacy experience is required. Prior work or student rotational experience may be considered as relevant.
  • Immunization certification
  • Current licensure by the Iowa Board of Pharmacy Examiners is required. Includes those eligible for obtaining this licensure.
  • Excellent written and verbal communication skills
  • Professional experience working effectively with individuals from a variety of backgrounds and perspectives.

Responsibilities

  • Provides Therapeutic Drug Monitoring (TDM) on clinic-administered medications and take-home prescription orders.
  • Completes a comprehensive patient assessment including evaluation of medications and medical history.
  • Evaluates prescription orders for appropriateness of indication, dose, route of administration, drug interactions, dose adjustments for organ dysfunction, physical and therapeutic incompatibilities, drug-induced side effects and adverse reactions, potential drug misuse or abuse, cost-effectiveness, and duplicate therapy.
  • Ensures pharmacokinetic assessment of drug dosing on targeted patients through clinical assessment and preparation of formal dosing recommendations.
  • Communicates with prescribers regarding patient medication needs.
  • Conducts patient profile reviews and updates as needed.
  • Provides appropriate drug information to other healthcare professionals.
  • Confirms/clarifies all patient drug allergies noted in the medical record.
  • Participates in collecting and documenting medication histories on patients.
  • Evaluates patient compliance with prescribed medication therapy and identifies potential problems.
  • Utilizes appropriate resources and coordinates the resolution of financial concerns with department financial counselors.
  • Documents all patient care activities performed, including patient drug allergies, height and weight, medication histories, medication counseling, interventions with prescribers and other healthcare providers, patient interactions (in clinic and remotely), drug dosing recommendations, Drug Use Evaluation Program activities, and steps taken to identify, evaluate, or avoid adverse drug reactions.
  • Educates patients on pharmaceutical care services, including medication history, drug allergy history, patient teaching classes, and medication counseling.
  • Communicates patient information to local healthcare providers for continuity of care.
  • Acts as a patient advocate for all concerns associated with drug therapy and influences therapeutic decisions.
  • Participates in hospital-wide programs to improve care, such as the Drug Use Evaluation Program and Adverse Drug Reaction Reporting Program.
  • Follows up on medication system problems, including unclear or inappropriate prescription orders, cost-effective drug therapy, and prescriptions for non-formulary products.
  • Carries out Medication Use Evaluation Program initiatives through prescriber intervention and education.
  • Serves on intra- and interdepartmental committees, task forces, and teams as required.
  • Facilitates completion of monthly clinic audits as assigned by the supervisor.
  • Serves as preceptor for pharmacy students and pharmacy residents.
  • Presents inservice educational programs to staff nurses, physicians, and other colleagues.
  • Presents at and/or participates in other educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UI Health Care or UI departments, and state and national organizations.
  • Conducts educational classes for pharmacy students and pharmacy residents.
  • Conducts patient education classes about the proper use of medications.
  • Serves as mentor and role model for other pharmacist practitioners within the department, particularly those who provide ambulatory care services.
  • Participates in the facilitation of patient transfer from inpatient to outpatient settings, including bedside delivery of discharge medications and patient education at the point of care.
  • May participate in research activities, including support for Investigational Drug Service prescriptions and orders.
  • Completes and maintains compliance with all competencies and educational requirements.
