Ambulatory Care Clinical Pharmacist - UVA Specialty Pharmacy (Hybrid)
About The Position
Help patients with complex and rare conditions begin and sustain specialty therapies safely, confidently, and affordably. In this high-impact clinical pharmacy role, your expertise will strengthen medication access, optimize therapeutic outcomes, and support continuity of care across multiple specialty service lines. Join UVA Specialty Pharmacy as a centralized ambulatory care clinical pharmacist supporting patients throughout their treatment journey. You will combine longitudinal medication management, patient education, clinical assessment, and interdisciplinary collaboration in a hybrid practice model designed for focused clinical work and professional growth. As part of an expanding specialty pharmacy program, you will manage a diverse patient population receiving specialty medications across areas such as oncology, rheumatology, digestive health, cardiology, pulmonary arterial hypertension, and other rare or complex conditions. This is not a clinic-embedded or dispensing role. Instead, you will practice as a centralized clinical specialist, connecting with patients telephonically and partnering virtually with providers, nurses, clinic teams, and medication access specialists. Your average day will balance patient counseling, clinical review, follow-up assessments, documentation, and collaboration. You will evaluate therapy appropriateness and safety, help patients understand what to expect from treatment, and monitor outcomes over time. The breadth of the practice offers continuous exposure to evolving specialty therapies and disease states, with opportunities to develop deeper specialization as the program grows.
Requirements
- Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.
- Virginia State Board of Pharmacy License or within 60 days of hire.
- American Heart Association (AHA) Health Care Provider BLS certification required.
Nice To Haves
- PGY-1 residency or equivalent clinical experience preferred.
Responsibilities
- Review specialty medication prescriptions for appropriateness, efficacy, accuracy, and safety
- Counsel newly referred patients on medication administration, potential adverse effects, therapy expectations, and disease state management
- Conduct longitudinal follow-up assessments focused on effectiveness, safety, adherence, and clinical outcomes
- Document patient progress and clinical interventions in Epic
- Maintain documentation aligned with ACHC and URAC accreditation standards
- Communicate clinical findings and recommendations to providers, nurses, and clinic teams through virtual channels
- Partner with medication access specialists to help address insurance, prior authorization, reimbursement, and affordability barriers
- Contribute to quality improvement, medication safety, medication use evaluation, REMS, and performance improvement initiatives
- Support the implementation of new specialty therapies and participate in clinical research opportunities
- Build trusted virtual relationships while independently managing a longitudinal patient population
