About The Position

The Specialty Pharmacy - Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. They apply specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, neurology, infectious disease, oncology, nephrology, or dermatology. Provides clinical, pharmacy, and medication expertise in each area including policy development, pharmacy operations, prior authorization support and medication therapy outcomes.

Requirements

  • Licensed pharmacist in the State of Vermont.

Nice To Haves

  • Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred.
  • 3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
