About The Position

Responsible for providing clinical pharmacy services for outpatient patients by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy. This is a part-time (40 hours per pay-period), salaried, Monday thru Friday (flexible days) position for a Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care, PCMH offices (part of the Trinity Health Saint Mary’s health-system in Grand Rapids, Michigan). Responsibilities include disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral-based Comprehensive Medication Review with recommendations to physicians, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team-based care within a multidisciplinary model. Additional activities include time in our Anticoagulation Management Service (coverage for PTO), and other multidisciplinary initiatives focused on particular high-risk patients or populations (e.g., depression, frail elderly) (approximately ½ day/week). Participation in the training of Trinity Health Grand Rapids PGY-1 & PGY-2 Pharmacy residents and PharmD students from the College of Pharmacy at Ferris State University is also expected as is participation in both medical group and/or health system committees.

Requirements

Doctor of Pharmacy degree (PharmD) from an ACPE-accredited school/college of Pharmacy (required)
PGY-1 Residency (required)
Licensed (pharmacist and controlled substance) in the State of Michigan, or eligible

Nice To Haves

PGY-2 Ambulatory Care Residency (strongly preferred)
Commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities
BPS Specialty Certification (Pharmacotherapy or Ambulatory Care, preferred; or eligible with certification within 1 year of hire)

Identifies and resolves potential and actual drug related problems
Assists physicians in designing dosing regimens based on patient-specific dosing parameters
Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines
Provides support to the Pharmacy & Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug therapy, etc.
Participates in performance improvement initiatives and research as appropriate
Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant
Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned
Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians
Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.
