About The Position

PharmD Live is seeking an experienced Clinical Pharmacist licensed in New Jersey to join our growing virtual care team. This role focuses on delivering pharmacist-led clinical services that help patients better manage chronic conditions, improve medication adherence, and achieve positive health outcomes. The successful candidate will work remotely while collaborating with providers, care coordinators, and interdisciplinary teams to deliver evidence-based, patient-centered care through innovative telehealth programs.

Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Active New Jersey Pharmacist License in good standing
  • Minimum of two (2) years of pharmacist experience in ambulatory care, clinical pharmacy, managed care, telehealth, or related settings
  • Strong computer proficiency and ability to navigate multiple clinical systems
  • Ability to work independently in a remote environment while maintaining productivity and quality standards
  • Willingness to obtain additional pharmacist licenses as business needs evolve
  • Ability to educate patients in Spanish language

Responsibilities

  • Conduct comprehensive medication reviews and medication reconciliation services
  • Identify, assess, and resolve medication-related problems
  • Provide telephonic and virtual patient consultations
  • Support patients with chronic conditions including diabetes, hypertension, heart failure, COPD, and other complex disease states
  • Develop individualized medication management recommendations for providers and patients
  • Promote medication adherence through patient coaching and motivational interviewing
  • Participate in care transition and post-discharge medication review activities
  • Document clinical encounters accurately and in accordance with regulatory and organizational standards
  • Collaborate with physicians, nurse practitioners, and care teams to optimize patient outcomes
  • Contribute to quality improvement initiatives and patient engagement efforts
