The Clinical Pharmacist at Atrium Health CarolinaCARE is responsible for overseeing pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. They utilize effective patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, and administration. The role involves designing, recommending, monitoring, and evaluating patient-specific pharmacotherapeutic regimens, implementing recommendations, and documenting appropriately. The pharmacist reviews and interprets medication orders, participates in medical emergencies, and actively supports medication safety and quality improvement initiatives. They also perform quality assurance and performance improvement of the medication-use system, collaborate with the healthcare team on formulary adherence and medication-use processes, and assist in acquiring specialized medications. Practice must comply with organizational policies, procedures, and local, state, and federal regulations. The pharmacist prepares, dispenses, and oversees the storage and handling of medications. Additionally, they serve as a preceptor for pharmacy students and residents, mentor and train pharmacy teammates, and direct the work of pharmacy support staff to ensure safe and efficient workflows. The role requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient needs based on age-specific requirements.
