About The Position

Join TriHealth as a Clinical Pharmacist Specialist. TriHealth is seeking an experienced Critical Care Clinical Pharmacist Specialist to join our team at Bethesda North Hospital in Cincinnati, OH. In this full-time, day-shift role, you will provide comprehensive pharmaceutical care to critically ill patients as a key member of our interdisciplinary intensive care teams. The specialist collaborates closely with physicians, nurses, and other healthcare professionals to support optimal patient outcomes within both the Medical-Surgical Intensive Care Unit (MSICU) and Cardiovascular Intensive Care Unit (CVICU), serving these teams on a rotating basis with other pharmacists. This position offers the opportunity to deliver advanced, patient-centered care while contributing to education, quality initiatives, clinical innovation, and the advancement of critical care pharmacy practice.

Requirements

• Bachelor's degree in pharmacy; PharmD or Bachelor of Science degree with equivalent decentralized hospital pharmacy experience (3years) OR accredited PGY-1, PGY-2 residency
• Pharmacist, Ohio Board of Pharmacy Licensure
• Clinical mastery of dedicated area of pharmacy practice that is consistent with the job posting
• 1-2 years’ Clinical Pharmacy practice that is consistent with the job posting
• 1-2 years' experience Clinical Pharmacy
• Completion of ASHP-accredited PGY1 pharmacy residency or Board Certification in related area of practice

Responsibilities

• Assures pharmacy is an integral part of the health-care delivery system, leading assigned patient service lines, clinical areas, and therapeutic programs; and facilitating pharmaceutical care services, direct patient care programs, and medication utilization systems.
• Designs and implements stewardship activities and restriction/surveillance programs.
• Tracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmarks against local and national best practices and identifies practice related issues.
• Develop and oversees policies and procedures.
• Delivers direct patient care and clinical practice, including decentralized and service-based programs, maintaining proficiency in decentralized pharmacy services and clinical pharmacy programs.
• Is an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care.
• Identifies high-risk patients and implements interventions.
• Makes appropriate evidence-based, patient-centered medication recommendations, and provides discharge medication review, reconciliation, and counseling as appropriate.
• Participates in the management of medical emergencies.
• Provides pharmaceutical services throughout medical center, maintaining proficiency in hospital computer systems and medication ordering systems.
• Provides accurate, safe, timely, and appropriate medication therapy.
• Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems and treatment failures.
• Communicates effectively and appropriately with healthcare providers and caregivers.
• Participates in new pharmacist orientation and contributes regularly to intra-departmental needs.
• Works closely with others to facilitate effective pharmacy education and competency evaluation.
• Participates in pharmacy operations and medication dispensing.
• Maintains proficiency in and actively engages in operational programs, central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas as appropriate for job assignment.
• Facilitates specialty medication procurement, ordering, and dispensing procedures including but not limited to chemotherapy, parenteral nutrition, controlled substances, etc. as appropriate for assignment.
• Oversee pharmacy operations and technician practice/activities
• Facilitates experiential education and practice advancement.
• Maintains proficiency in preceptor roles and actively emulates education/mentorship skills.
• Develops student, resident, and staff training experiences/competences and creates new relationships for teaching and training opportunities.
• Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs
• Communicates relevant information to other department members.
• Assumes personal responsibility for professional development, particularly in ongoing clinical education.
