About The Position

Bring your passion to Texas Health so we are Better + Together. This position highlights a Clinical Pharmacist role in Ambulatory Care, located at 1301 Pennsylvania Ave. Fort. Worth, TX 76104. The role is full-time, 40 hours per week, with a day shift schedule. The Pharmacy Department is described as top-tier with a culture of excellence, operating in a high acuity, high volume, fast-paced environment that provides a full range of patient care services. The department supports academic programs including interprofessional teaching services (internal medicine, cardiology, surgical/trauma, critical care), PGY1 and PGY2 pharmacy residency programs, and student precepting. The leadership and team are supportive, promoting continuous learning and professional development.

Requirements

  • Bachelor's Degree Pharmacy Required
  • Other Pharm D. Required
  • CLIAFE Must comply with CLIA regulations regarding foreign education equivalency required.
  • PHAR- Pharmacist upon hire required.
  • Must be proficient in written and verbal English language communication skills, computer proficient and possess basic math skills.

Nice To Haves

  • 3 years of hospital experience or completion of residency preferred.
  • Completion of PGY1 training is required. PGY2 training in Ambulatory Care is strongly preferred.
  • Infectious Disease experience is highly preferred
  • Board certification (BCPS or BCACP) within 30 days of hire is preferred.
  • Medical staff credentialing and privileging must be completed within 90 days of hire (preferred).
  • Experience working with HIV patient populations Preferred.
  • Malpractice insurance coverage is preferred.

Responsibilities

  • Communicating effectively, building positive professional relationships and role modeling teamwork.
  • Timely and accurate support of medication distribution including medication order verification and oversight of pharmacy technician responsibilities.
  • Provision of clinical pharmacy services for the safe and efficient patient care.
  • Ensuring the accuracy of one's own work as well as maintaining required competencies and other documentation. Consistent support of the department's quality improvement and patient safety standards programs and initiatives is also required.
  • Ongoing professional development and growth.
  • Not only the management of one's own time but shares the responsibility for minimizing pharmaceutical costs (including wastage) and discovering new process efficiencies.
  • Respond to medical emergencies per hospital policy as available.
  • Other duties as assigned.
