About The Position

The Clinical Ambulatory Pharmacist serves as an integral member of the pediatric ambulatory care team, collaborating closely with providers, patients, caregivers, and pharmacy staff to optimize medication therapy outcomes. This role focuses on medication management, patient education, specialty medication access, and care coordination across multiple pediatric specialty clinics. This position is assigned to multiple pediatric specialty clinics and offers the opportunity to build strong collaborative relationships with providers, patients, caregivers, outpatient pharmacy teams, and specialty pharmacy services to improve medication access, education, and overall patient care.

Requirements

  • Graduate from an American Council on Pharmacy Education accredited school of pharmacy
  • PGY1 residency accredited by the American Society for Health-System Pharmacy or 2 years commensurate pharmacist experience in a health-system setting
  • Texas Pharmacist License or eligible for Texas Licensure as a registered pharmacist
  • Pharmacist and pharmacy preceptor licensure must be obtained within 90 days of employment
  • Current Basic Life Support for Healthcare Providers certification.
  • Pediatric Advanced Life Support certification within 60 days of hire (critical care areas only)

Nice To Haves

  • Previous ambulatory care pharmacy experience preferred
  • Experience working with pediatric patients and specialty medications is highly desirable
  • Strong patient counseling, communication, and interdisciplinary collaboration skills

Responsibilities

  • Provide comprehensive medication education and counseling to pediatric patients and caregivers.
  • Collaborate with multidisciplinary providers to optimize medication therapy and improve patient outcomes.
  • Support medication access initiatives, including prior authorizations and appeals, financial assistance programs, and specialty medication coordination.
  • Work closely with outpatient pharmacy and specialty pharmacy teams to facilitate medication procurement, adherence, and ongoing therapy management.
  • Monitor and manage patients receiving specialty therapies, ensuring appropriate follow-up and medication utilization.
  • Serve as a clinical resource for providers regarding medication selection, dosing, monitoring, and therapeutic alternatives.
  • Participate in quality improvement initiatives and contribute to enhancing patient care processes within ambulatory clinics.
