About The Position

The Clinical Pharmacist Ambulatory Care position at Texas Health Fort Worth is a full-time, day shift role within a top-tier pharmacy department known for its culture of excellence. This high-acuity, high-volume, fast-paced environment provides a full range of patient care services. The department supports academic programs including interprofessional teaching services (internal medicine, cardiology, surgical/trauma, critical care), PGY1 and PGY2 pharmacy residency programs, and student precepting. The role involves delivering high-quality patient care through nursing education, research, and innovations in practice, supported by a strong Unit Based Council (UBC) and a supportive, team environment with outstanding opportunities for growth. Requirements

Bachelor's Degree Pharmacy Required
Other Pharm D. Required
CLIAFE Must comply with CLIA regulations regarding foreign education equivalency required.
PHAR- Pharmacist upon hire required.
Must be proficient in written and verbal English language communication skills, computer proficient and possess basic math skills.

Nice To Haves

3 years of hospital experience or completion of residency preferred.
Completion of PGY1 training is required.
PGY2 training in Ambulatory Care is strongly preferred
Infectious Disease experience is highly preferred
Board certification (BCPS or BCACP) within 30 days of hire is preferred.
Medical staff credentialing and privileging must be completed within 90 days of hire (preferred).
Experience working with HIV patient populations Preferred
Malpractice insurance coverage is preferred.

Responsibilities

Communicating effectively, building positive professional relationships and role modeling teamwork.
Timely and accurate support of medication distribution including medication order verification and oversight of pharmacy technician responsibilities.
Provision of clinical pharmacy services for the safe and efficient patient care.
Ensuring the accuracy of one's own work as well as maintaining required competencies and other documentation.
Consistent support of the department's quality improvement and patient safety standards programs and initiatives.
Ongoing professional development and growth.
Management of one's own time and shares the responsibility for minimizing pharmaceutical costs (including wastage) and discovering new process efficiencies.
Respond to medical emergencies per hospital policy as available.
Other duties as assigned.
