About The Position

The Clinical Pharmacist Ambulatory Care position at Texas Health Fort Worth is a full-time, day shift role within a top-tier pharmacy department known for its culture of excellence. This high-acuity, high-volume, fast-paced environment provides a full range of patient care services. The department supports academic programs including interprofessional teaching services (internal medicine, cardiology, surgical/trauma, critical care), PGY1 and PGY2 pharmacy residency programs, and student precepting. The role involves delivering high-quality patient care through nursing education, research, and innovations in practice, supported by a strong Unit Based Council (UBC) and a supportive, team environment with outstanding opportunities for growth. Texas Health Fort Worth is an 851-bed, Magnet-designated, full-service hospital serving the Fort Worth community since 1930, offering advanced care in various specialties and holding multiple certifications and recognitions.

Requirements

  • Bachelor's Degree Pharmacy Required
  • Other Pharm D. Required
  • CLIAFE Must comply with CLIA regulations regarding foreign education equivalency required.
  • PHAR- Pharmacist upon hire required.
  • Must be proficient in written and verbal English language communication skills, computer proficient and possess basic math skills.

Nice To Haves

  • 3 years of hospital experience or completion of residency preferred.
  • Completion of PGY1 training is required.
  • PGY2 training in Ambulatory Care is strongly preferred
  • Infectious Disease experience is highly preferred
  • Board certification (BCPS or BCACP) within 30 days of hire is preferred.
  • Medical staff credentialing and privileging must be completed within 90 days of hire (preferred).
  • Experience working with HIV patient populations Preferred
  • Malpractice insurance coverage is preferred.

Responsibilities

  • Communicating effectively, building positive professional relationships and role modeling teamwork.
  • Timely and accurate support of medication distribution including medication order verification and oversight of pharmacy technician responsibilities.
  • Provision of clinical pharmacy services for the safe and efficient patient care.
  • Ensuring the accuracy of one's own work as well as maintaining required competencies and other documentation.
  • Consistent support of the department's quality improvement and patient safety standards programs and initiatives.
  • Ongoing professional development and growth.
  • Management of one's own time and shares the responsibility for minimizing pharmaceutical costs (including wastage) and discovering new process efficiencies.
  • Respond to medical emergencies per hospital policy as available.
  • Other duties as assigned.
