About The Position

Eliot's Homeless Services Division is seeking a Clinical Outreach Navigator to join our Community Support Team which provides housing stabilization support to unsheltered adults who are in transition to housing pre- and post- move in. Eliot is committed to a Housing First Model that prioritizes permanent housing as the initial step in assisting individuals experiencing homelessness. The ideal candidate for the Clinical Outreach Navigator is a master's-level professional with a strong commitment to mental health recovery and a background in working with adults experiencing severe mental illness or homelessness. This individual excels at building supportive relationships, is skilled in providing community-based care and collaborating effectively with a multidisciplinary team. They will provide direct clinical support, crisis response, and harm reduction counseling.

Requirements

  • Master’s degree in related fields
  • Experience working with adults with severe and persistent mental illness or homelessness.
  • Ability to provide flexible services that are community based and focused on team work.
  • Must have a valid driver’s license and reliable transportation.

Responsibilities

  • Provides direct clinical support to assigned individuals including the provision of harm reduction counseling/education and appropriate supplies to mitigate risk of overdose
  • Provides weekly clinical support to program staff and clients.
  • Consult with multidisciplinary team to support identified clients with complex behavioral health profiles, substance use disorders, and complex medical illnesses with appropriate resources and strategic interventions.
  • Support team in all aspects of housing placement, transition and orientation
  • Responds to crisis/high risk situations collaboratively with leadership and field personnel
  • Establish and maintain collaborative relationships with outside agencies.
  • Meet with high risk clients on the street within BOS team, in treatment programs, hospitals and houses of correction in order to assess, establish and maintain continuity of care.
