About The Position

This role focuses on improving patient outcomes through collaboration with physicians, allied health providers, and the care transitions team. The Clinical Nurse Outcomes Coordinator will facilitate the development, implementation, and assessment of processes impacting patient care, utilizing benchmark data to identify areas for improvement in care and patient safety. The position also involves incorporating the quality caring model and participating in shared decision-making through nursing councils.

Requirements

  • Master’s Degree in Nursing Required
  • Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire
  • Two (2) years direct patient care (RN) experience Required
  • Five (5) years current hospital experience Required
  • RN – Registered Nurse Upon Hire Required
  • BCLS – Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required
  • CPI - Crisis Prevention Intervention Training Maintained Annually 90 Days Required
  • Other CNL or other professional nursing certification 12 Months Required

Responsibilities

  • Collaborates with physicians and/or APRN for services needed as well as all with other allied health care providers
  • Partners with physicians by engaging in rounding to discuss priority of patients and patient care needs
  • Participates in interprofessional rounds, leading when appropriate
  • Collaborates with care transitions team to facilitate discharges, RCB measures, review of excess days and patient throughput
  • Facilitates the development, implementation, monitoring, and assessment of processes that impact patient outcomes
  • Uses benchmark data from internal and external sources to identify opportunities for improvement of care and patient safety
  • Incorporates the quality caring model in daily nursing decisions
  • Participates in shared decision making by participating in councils as requested by nursing leadership
