About The Position

Lifesong Hospice, a subsidiary of Country Meadows Retirement Communities, is hiring a full-time, day-shift Clinical Nurse Liaison (RN). The role connects with clients and families, including in home settings, and supports symptom management, quality of life, and peace of mind. The liaison works with acute care facility referral partners and hospice teams to manage referrals, evaluate patients, explain hospice services, and coordinate admissions and transitions to appropriate care settings. Lifesong personalizes hospice care plans to clients’ preferences and needs, helping them transition as comfortably as possible while honoring the meaning and legacy of each unique life. The role offers direct access to a support team, clinical training and mentorship, and a dedicated weekend and evening on-call team. Benefits may vary based on employment status.

Requirements

Current Pennsylvania licensure as a registered nurse (RN).
Graduate of a National League for Nursing-accredited school of nursing.
Minimum of two years of experience; experience in hospice nursing.
Self-directed and able to work with minimum supervision.
Demonstrated ability to prioritize tasks and handle pressure and delicate situations.
State driver’s license in good standing and an automobile maintained in good working order.
High proficiency with computers, Microsoft Office, cellular technology, and EMR.
Able to perform the physical tasks needed in this role.

Responsibilities

Maintain regular contact with acute care facility referral partners, including discharge planners, case managers, physicians, hospitals, nursing staff, and other appropriate clinical personnel.
Receive, track, and follow through on hospice referrals from acute care facilities.
Evaluate patients and coordinate or complete the admission process, as assigned.
Explain hospice services and care options to patients and families, and complete required admission documentation.
Coordinate with hospital case managers and hospice teams to support timely admission, safe discharge, and transition to an appropriate setting.
