To provide direct and indirect nursing services to patients and families. This role involves clinical advancement, utilizing systematic planning and timely exchange of information with the multidisciplinary team and patient/family to coordinate care. The nurse will demonstrate the ability to focus on and manage multiple patient needs and aspects of care, actively communicating with the care team to plan patient care and utilizing available resources to problem-solve. Clinical judgment and decision-making are key, employing the nursing process and critical thinking to provide individualized, goal-directed patient care consistent with current safety standards. The role includes identifying actual/potential problems, continuously evaluating interventions, making appropriate referrals, and collaborating with patients, families, and team members to develop comprehensive care plans. The nurse will also act as a preceptor/teacher for other nurses and implement safe, therapeutic, and efficient care for patients with complex needs, documenting per department guidelines and serving as a resource for documentation issues. Clinical leadership involves delegating and assigning care delivery processes, assessing physician orders, assisting coworkers, and looking for ways to improve work processes. Establishing a therapeutic relationship with patients and families, anticipating problems, analyzing social/cultural variables, and providing age-appropriate patient education are also essential. The nurse will coordinate transfer and discharge planning and teaching, collaborating with other team members to develop patient education materials and demonstrating flexibility in teaching based on learner needs and adult learning principles.
