About The Position

BAYADA HOME HEALTH CARE is seeking a Registered Nurse (RN) Clinical Manager for our Long Island Nassau Assistive Care office in Garden City, NY. This office provides compassionate, high-quality personal care and support to clients throughout Nassau County, helping them remain safely and comfortably at home. As a Clinical Manager, you’ll lead and support field staff, provide clinical oversight, strengthen client relationships, and help ensure consistent, high-quality care delivery throughout the community.

Requirements

• Current Registered Nurse (RN) license in good standing in New York.
• Minimum of two years of RN experience.
• Strong clinical assessment, organization, communication, and problem-solving skills.
• Ability to build trusting relationships with clients, families, caregivers, and colleagues.
• Passion for supporting clients’ independence, dignity, and quality of life at home.
• Ability to travel throughout Nassau County for client visits and community engagement.

Nice To Haves

• Prior supervisory or leadership experience preferred.
• Home care, personal care, or community-based healthcare experience preferred.

Responsibilities

• Lead, inspire, and support Home Health Aides and other field staff in delivering exceptional care.
• Visit clients throughout Nassau County and conduct assessments and supervisory visits.
• Develop individualized care plans and oversee their implementation.
• Monitor client needs, service delivery, and changes in condition.
• Supervise, mentor, and coach caregivers in the field.
• Facilitate orientation for newly hired staff.
• Conduct competency assessments for new and tenured employees.
• Partner with families, caregivers, referral sources, and office leadership to support continuity of care.
• Promote compliance with BAYADA policies, applicable regulations, and documentation standards.
• Participate in quality improvement initiatives and help resolve client or employee concerns.
• Model The BAYADA Way through compassionate leadership, clear communication, and dependable follow-through.
