About The Position
Riverside Home Health & Hospice is seeking an experienced Home Health RN to serve as a Clinical Manager. This role involves mentoring clinicians, elevating patient care, and building a supportive and inspiring culture. The Clinical Manager is crucial for ensuring exceptional patient care by supporting the clinicians who provide it daily. This position directly influences clinical quality, patient outcomes, OASIS accuracy, hospitalization reduction, regulatory compliance, clinician development, and fostering a culture of accountability and continuous improvement. The ideal candidate is passionate about developing people, improving quality, and leading with integrity.
Requirements
- Active Oregon Registered Nurse (RN) license
- Current BLS certification (AHA)
- Previous Home Health experience required
- Experience with Homecare Homebase (HCHB) required
- Strong OASIS knowledge and experience required
- Previous clinical leadership or supervisory experience required
- Excellent communication, coaching, and organizational skills
- Valid driver’s license and insured vehicle
- Strong clinical judgment and a passion for developing others
Responsibilities
- Reviewing clinician OASIS accuracy and provide feedback to clinicians
- Supporting clinicians with complex patient situations
- Monitoring quality measures, regulatory compliance, and patient outcomes
- Assisting with case conferences, care planning, and clinical decision-making
- Collaborating with physicians, intake, scheduling, and operational leaders
- Helping drive initiatives that improve patient satisfaction, quality metrics, and hospitalization rates
