Requirements

  • Current Oregon CNA license
  • CPR Certification
  • Valid driver’s license and reliable transportation
  • Strong communication and observation skills
  • Ability to work independently in the field

Responsibilities

  • Provide personal, hands-on care in patients’ homes
  • Help patients maintain dignity, comfort, and quality of life
  • Support families during emotional and meaningful moments
  • Be the eyes and ears of the care team, identifying changes in condition
  • Help patients remain safely and comfortably at home
  • Providing personal care (bathing, dressing, toileting, hygiene)
  • Assisting with mobility and range of motion exercises
  • Taking and recording vital signs
  • Maintaining a clean and safe patient environment
  • Reporting changes in condition to the nurse or case manager
  • Supporting patients and families with compassion and respect
