Requirements
- Current Oregon CNA license
- CPR Certification
- Valid driver’s license and reliable transportation
- Strong communication and observation skills
- Ability to work independently in the field
Responsibilities
- Provide personal, hands-on care in patients’ homes
- Help patients maintain dignity, comfort, and quality of life
- Support families during emotional and meaningful moments
- Be the eyes and ears of the care team, identifying changes in condition
- Help patients remain safely and comfortably at home
- Providing personal care (bathing, dressing, toileting, hygiene)
- Assisting with mobility and range of motion exercises
- Taking and recording vital signs
- Maintaining a clean and safe patient environment
- Reporting changes in condition to the nurse or case manager
- Supporting patients and families with compassion and respect
