About The Position

BAYADA HOME HEALTH CARE is seeking a full-time Clinical Manager (RN) – Hospice for our PA Boyertown Hospice office in Wyomissing, PA. This team supports clients and families across Berks, Lehigh, and Northampton counties, providing compassionate end-of-life care focused on comfort, dignity, and peace. Requirements

  • Current RN license in good standing in Pennsylvania.
  • Graduate of an accredited and approved nursing program; BSN preferred where applicable.
  • Hospice experience.
  • Previous clinical management or other relevant leadership experience.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to lead with initiative, adaptability, and sound clinical judgment.
  • Comfort supporting and managing a multidisciplinary field team.

Nice To Haves

  • Home care experience.
  • Experience leading or participating in IDG meetings.
  • Experience reviewing referrals, hospital documentation, and managing clinical caseloads.
  • Proven ability to build trust, collaborate effectively, and contribute positively to team culture.

Responsibilities

  • Lead and support field staff including RNs, CNAs, home health aides, and other interdisciplinary team members to ensure high-quality hospice care.
  • Respond to and help manage clinical calls from nurses in the field.
  • Review referrals, hospital notes, and other clinical documentation to support safe and appropriate care planning.
  • Lead IDG meetings and partner with the interdisciplinary team to coordinate care.
  • Oversee and manage caseloads in a dynamic hospice environment, adjusting to changes in admissions and discharges.
  • Provide proactive, flexible leadership to support staff and address day-to-day operational and clinical needs.
  • Collaborate with office and clinical leaders to ensure strong service delivery and team alignment.
  • Foster a positive team environment through strong communication, partnership, and hands-on support.
