Requirements

  • Current unrestricted North Carolina Registered Nurse (RN) license
  • Minimum of two years of nursing experience
  • Previous leadership or supervisory experience
  • Excellent communication and interpersonal skills
  • Strong organizational and problem-solving abilities
  • Valid driver's license and reliable transportation

Nice To Haves

  • Home care, hospice, geriatrics, or community health experience
  • Experience with electronic health records (WellSky preferred)
  • Experience with quality improvement and regulatory compliance
  • Passion for coaching, mentoring, and developing others

Responsibilities

  • Lead the agency's clinical operations
  • Complete and oversee client nursing assessments
  • Develop, review, and approve individualized care plans
  • Monitor changes in client condition and coordinate appropriate interventions
  • Provide clinical consultation to office staff and caregivers
  • Lead Quality Assurance and Performance Improvement initiatives
  • Investigate client complaints, grievances, and incidents
  • Complete state-required documentation and reporting
  • Conduct chart audits and regulatory record reviews
  • Ensure compliance with North Carolina Home Care Licensure Rules and agency policies
  • Conduct new caregiver orientation
  • Validate caregiver competencies and skills
  • Administer TB testing
  • Coordinate annual competencies
  • Lead ongoing caregiver education and weekly skills training
  • Monitor online training compliance through Care Academy
  • Serve as the agency's clinical resource
  • Answer caregiver and client clinical questions
  • Coach and mentor caregivers
  • Collaborate with scheduling staff regarding client assignments and care needs
  • Communicate with physicians, hospice providers, hospitals, home health agencies, PACE, GUIDE affiliates, and other referral partners
  • Coordinate ancillary services and community resources
  • Represent the agency professionally within the healthcare community
  • Participate in daily leadership meetings
  • Assist with policy development
  • Monitor quality indicators and agency performance metrics
  • Support strategic initiatives and agency growth
