Kindred Hospital Albuquerque is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Our hospital provides both intensive care and telemetry-level services, offering advanced monitoring and support for patients requiring extended medical care. With a multidisciplinary team dedicated to personalized treatment plans, we focus on helping patients achieve significant recovery and transition to the next level of care. This role conducts patient assessments to identify appropriate patients for potential admission into the system. It involves daily follow-up with patients and their decision-makers for consistent clinical reassessment. The position is responsible for managing referrals and admissions generated through assigned hospitals and territories, representing the company with professionalism, and maintaining positive relationships with accounts. The Clinical Liaison designs and manages marketing strategies, identifies and contacts new referral sources, and conducts in-person educational meetings. This role acts as a liaison by coordinating clinical and benefit information between patients, decision-makers, referral sources, and the specialty hospitals served. A significant portion of the work (80% or more) is performed outside the home, with the expectation of being on-site at respective accounts daily.
