About The Position

Select Specialty Hospital is seeking a Per-Diem Clinical Liaison (RN/RRT) for their Critical Illness Recovery Hospital (LTACH). This role involves advocating for referred patients during the pre-admission process by collaborating with clinical teams to assess patient needs. The liaison will serve as a resource for healthcare professionals by fostering relationships within medical facilities. Utilizing clinical expertise and problem-solving skills, they will coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialized post-ICU care. The work schedule is on an as-needed basis for weekend coverage, PTO, and medical leave coverage.

Requirements

Current and valid state licensure or eligibility for state licensure as a Registered Nurse or Respiratory Therapist.
Demonstrated computer proficiency.
Must exercise sound judgment in handling the professional/confidential nature of healthcare sales.

Nice To Haves

Two years of related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role.
Demonstrated competency with the preadmission process, including assessments and relationship building in a rehabilitation setting or in another post-acute care setting.
Proven track record in sales and marketing with at least one year of experience in a related position.

Responsibilities

Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
Coordinate care plans with physicians and families.
Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process by removing payer-related and operational barriers for patients getting into our highly effective level of care.
