About The Position

Adoration Home Health and Hospice is seeking a Clinical Liaison to join their team. This position offers an opportunity to grow your skillset while providing essential one-on-one care to individuals in the home care setting. The role involves supporting patient transitions to skilled private duty services in their home environment, with secondary support for home health services. The Clinical Liaison will develop strong relationships with physicians, community health facilities, hospitals, and skilled nursing facilities within their assigned territory to improve the continuation of care through fostering a smooth transition for patients.

Requirements

  • Licensed Practical Nurse (LPN), Respiratory Therapist, Certified Respiratory Therapist (CRT) or Registered Respiratory Therapist (RRT) licensure in the applicable state preferred.
  • Minimum of two (2) or more years of clinical experience.
  • A demonstrated history of working in a cross-collaborative environment.
  • Excellent presentation, negotiation and relationship-building skills.
  • Self-motivated with the ability to work independently with minimal supervision and a flexible schedule.
  • Microsoft Office knowledge (Word, Excel, PowerPoint) preferred.
  • Ability to travel locally in the assigned territory.
  • Reliable transportation, a valid driver’s license and current automobile insurance.

Responsibilities

  • Manage communication channels between physicians, social workers, discharge planners, hospital case management and agency staff by ensuring all are aware of referral source request and concerns.
  • Communicate information, questions, and status reports from the patient care staff to the referral source.
  • Establish and maintain a system for handling non-admits and communicating this information to the referral source.
  • Coordinate and communicate care needed and relays concerns of physician and staff prior to admission or resumption of care for a patient to the agency staff and during course of treatment.
  • Coordinate clinical orders from the facility or physician regarding patient care and communicates any needs to agency staff.
  • Create and maintain a strong collaborative relationship with facility and physician staff.
  • Facilitate patient referrals for home health & skilled private duty admission, if requested, prior to the patient’s discharge from the facility.
  • Primarily conduct pre-discharge facility visits at the physician’s request to determine the need or eligibility.
  • Responsible for notifying a patient’s primary care, or specialists, about discharge from facility and upcoming admission to home health.
  • Manage assigned accounts by identifying opportunities for clinical alignment to improve clinical outcomes.
  • Provide education and training to clinical providers on the clinical capabilities of home health and skilled private duty services.
  • Inform hospital or physician personnel, patient and/or family of patient acceptance and documents all demographic, clinical, payer and other information as required on company approved forms to support diagnosis and home health care or skilled private duty orders.
  • Assist branch in timely processing of physician orders.
  • Foster an environment of effective communication and collaboration with branch staff and sales resources via weekly meetings.
