About The Position

Clinical Liaison (RN / RRT) for Select Specialty Hospital Critical Illness Recovery Hospital (LTACH) PRN / per diem. This role involves advocating for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. The liaison serves as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem-solving skills, they coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialized post-ICU care. The position offers a competitive salary plus monthly bonus incentives and an extensive onboarding & training program.

Requirements

Active RN or RT license required.
Ability to complete clinical assessments of medically complex patients.

Nice To Haves

Superior communication and relationship-building skills.
Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.

Responsibilities

Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process by removing payer-related and operational barriers for patients getting into our highly effective level of care.
Serve as a resource for healthcare professionals through fostering relationships within medical facilities.
Coordinate care plans with physicians and families.
Facilitate patient transfers.
Address barriers to accessing specialized post-ICU care.
