New England Life Care currently has an opening for a Clinical Liaison Teach Nurses at Brigham and Women's Hospital (BWH). This is a full-time, salaried, on-site position. You are eligible for quarterly commissions after your initial training period, and there is a generous differential if you must work any weekend shifts.

Requirements

Associate degree or diploma in Nursing
Valid driver’s license.
Excellent driving record.
Excellent patient assessment skills.
Ability to communicate effectively with all customers and members of the healthcare team verbally and in writing.
Good interpersonal skills.
Good organization skills.
Ability to work independently as well as a part of a team.
Dependability in attendance and job performance.

Nice To Haves

BS in Nursing preferred.
Intravenous therapy experience in hospital or homecare preferred.
Oncology experience preferred.
CRNI preferred.
Current BLS Certification preferred.
R.N. license in good standing in the state(s) where providing care / services preferred
Knowledge of HIPAA Privacy and Security Regulations preferred.
Knowledge of The Joint Commission accreditation process and overall JC standards preferred.
Working / practical knowledge of home infusion therapy administration / equipment use preferred.
Working / practical knowledge of microcomputer systems including patient documentation systems, Microsoft Office (Word, Excel, Outlook).

Responsibilities

Assesses needs and abilities of the patient and caregiver. Participates in developing a patient specific training plan and plan of care. Participates in patient training when appropriate.
Facilitates the discharge process by relaying orders and documentation necessary for discharge to the appropriate NELC departments.
Acts as clinical resource between NELC and all appropriate departments at the hospital. This includes, but is not limited to, the medical staff, care coordination/ case management, social services, pharmacy and nursing departments.
Assists in coordinating services with other home health agencies.
Monitors active and pending NELC patient status while hospitalized and provide ongoing communication re: patient status to appropriate NELC staff.
Comprehensively documents all interactions with patient and members of the healthcare team in the patient record and/or electronic progress notes.
Effectively communicates patient requirements and changes in patient status to appropriate members of infusion and reimbursement teams as well as the HHA staff involved in meeting the patient’s home health needs.
Participates in daily patient care meetings and case conferences.
Ensures that confidentiality of patient information is maintained in accordance with state and HIPAA Privacy and Security regulations.
Accurate completion and reimbursement determination of therapy specific reimbursement determination check lists such as Medicare TPN, Medicare ENT, Medicare Cardiac, etc. as determined by payer or NELC guidelines.
