About The Position

Via Elegante Assisted Living & Memory Care is seeking a confident, compassionate clinical leader to serve as their Clinical Director | Care Concierge. This role involves leading clinical operations while acting as a trusted advocate and care coordinator for residents and their families. In partnership with Centered Care, the Clinical Director will utilize proactive care practices, technology, and clinical insight to identify needs early and ensure residents receive timely and appropriate support. The position offers the opportunity to lead and develop a clinical team, oversee resident care plans, build relationships with various healthcare stakeholders, coordinate care, lead team meetings, oversee medication management and quality initiatives, coach staff, support specialized care needs, and participate in an on-call rotation.

Requirements

• Active, unrestricted Arizona RN, LPN, or LVN license.
• 3+ years of nursing experience in assisted living, memory care, skilled nursing, home health, or comparable senior care.
• 1+ year of clinical leadership or supervisory experience.
• 1+ year of hands-on dementia care and medication administration experience.
• Experience with EHR documentation and eMAR systems.
• Current CPR and First Aid certification.
• Valid Arizona fingerprint clearance card or ability to obtain one within required timelines.
• Valid driver's license and access to a private vehicle for business use.

Responsibilities

• Lead and develop a high-performing clinical team.
• Oversee individualized resident care and service plans.
• Build strong relationships with residents, families, physicians, and healthcare partners.
• Coordinate care with physicians, specialists, pharmacies, labs, home health, hospice, and therapy providers.
• Lead interdisciplinary team meetings and care conferences.
• Oversee medication management, clinical documentation, and quality initiatives.
• Coach and educate caregivers and clinical team members.
• Support dementia care, resident transitions, and complex healthcare needs.
• Serve as a clinical leader during urgent situations and participate in the director on-call rotation.
