About The Position

The Clinical Manager, under the supervision of the Director of Personal Care, assumes leadership and responsibility for planning, directing, coordinating, and implementing resident-focused care and services in the Personal Care Units. The Clinical Manager serves as a liaison between the Director of Personal Care, team members, physicians, and residents. They are authorized to act in the absence of the Director of Personal Care.

  • Coordinating care planning conferences by ensuring timely completion of assessments and support plans per policy and regulation
  • Educating team members on the contents of the support plans, and ensuring interventions are carried out as planned
  • Assisting the Director of Personal Care with conducting team member meetings regularly, and as needed
  • Assisting the Resident Assessment Committee in determining the appropriate level of care by providing pertinent resident information
  • Acting as a role model and resource for team members
  • Working well with team members and providing assistance as needed
  • Participating in projects as requested
  • Responsibility for or assisting Director of Personal Care as needed with payroll, scheduling of employees, and evaluations of Nurses and Certified Nursing Assistants
  • Providing direction and leadership in Quality of Life Meetings
  • Clinical oversight of resident assessments
  • Developing and implementing service plans
  • Communicating outcomes with resident and/or family members as required by DHS
  • Ensuring the psychosocial and spiritual needs of residents are met
  • Assuring team members use appropriate infection control practices
  • Instituting appropriate emergency interventions
  • Completing audits and observations to ensure compliance with infection control and other resident care regulations
  • Conducting regular audits of medication records, medication storage areas and medication administration records
  • Coordinating resident assessments with social worker, the Admissions department, and the administrative team
  • Ensuring that all admissions documentation is completed
  • Providing oversight of Nurses with admissions process
  • Providing all annual required documentation including, but not limited to, the resident’s initial assessment, support plan, med evaluation and Self-Medication Administration assessments
  • Monitoring documentation of resident care and services for accuracy, content, timeliness, and clinical pertinence
  • Updating resident support plans as appropriate
  • Assisting with new resident orientation, reviewing resident handbook, and tour of facility
  • Reviewing all incident/accident reports for completeness before they are given to the Director of Personal Care
  • Reviewing interventions/findings with team members and ensuring completion of documentation
  • Continuously assessing and improving the performance of outcomes of care and services provided by the nursing units
  • Identifying residents at risk for falls, pressure ulcers, incontinence, aspiration pneumonia and implementing preventative measures
  • Monitoring care to assure the provision of the highest possible quality of care
  • Monitoring resident care to assure the care being provided assists the resident to be as independent as possible
  • Acting as an advocate for the resident
  • Reporting all complaints voiced by residents
  • Providing written responses to concerns of residents and/or family members in accordance with facility policy
  • Collecting statistical data to aid in the determination of nursing care needs
  • Developing positive relationships with resident’s family members/significant others
  • Offering suggestions to improve resident care and facility practices at care plan and team meetings
  • Performing quality improvement audits of clinical records and areas cited in the DHS violation reports to ensure compliance with the plan of correction
