About The Position

Cornerstone Family Healthcare is actively recruiting for a Clinical Coordinator to join our growing team in Middletown, NY. This full-time position provides oversight of the nursing and clinical care provided in the department.

Requirements

  • Successful completion of LPN program from an Accredited School
  • Travel to Port Jervis and Pine Bush
  • BLS Certification
  • NYS Education Department Licensed Practical Nurse

Responsibilities

  • Provide daily oversight to the department’s LPNs and Medical Assistants.
  • Provide supervision to the department’s LPNs and Medical Assistants.
  • In collaboration with the Clinical Training Coordinator and Lead nurse, create, implement and supervise the new nurse/MA orientation process.
  • Ensure that nursing staff adhere to CORNERSTONE nursing policies.
  • Participates in CORNERSTONE’s Care Team Model including but not limited to Pre-Visit Planning, leading care team huddles/communication, directing population management activities for assigned patient panel, and directing and participating in quality improvement activities within the care team and department.
  • Manage patient flow based on the volume of patients in the department and the needs of each provider.
  • Coordinate the departmental nursing schedule in accordance with providers schedules, assuring adequate coverage at all times.
  • Complete daily assignments of nurses and medical assistants to providers and schedule lunch breaks.
  • Ensure the DEPARTMENT PROXY in Centricity is assigned and addressed in a timely manner and is free of outstanding phone notes/messages and orders concerning laboratory follow up from the Providers.
  • Ensure the department is stocked with all necessary equipment, supplies and forms.
  • Ensure that inventory is maintained for all drugs and biological and log their use as required by CORNERSTONE policies and procedures.
  • Ensure that inventory of sample medications is maintained and required logbooks are completed and up to date.
  • Ensure that all medication, both stock and samples, are inventoried on a monthly basis to check for expiration dates and recalls.
  • Monitors the crash cart for daily and monthly inspections and ensures that it is locked and fully stocked.
  • Receive all laboratory and diagnostic procedure results and promptly address any follow up needed according to Cornerstone policy.
  • Answer and respond to nurse telephone calls including but not limited to triage/clinical advice, prescription refills, requests form hospitals and other agencies, and general patient requests.
  • Provides triage of walk-in patients, under the supervision of the provider or Lead Nurse.
  • Completes patient screening and provider assistance including but not limited to; vital signs and chief complaints, assisting providers with in-office procedures, administration of medication and treatments according to provider orders, vaccinations, hearing and vision screenings, and patient education and instruction.
  • Responsible for carrying out Medical Providers orders for the following actions: EKG’s, Emergency response, medication administration not limited to Oral Medications, vaccinations, prescribed treatments and other procedures according to Provider orders, ensures that all CLIA waived testing performed on site has been quality controlled and logged in master QC book.
  • Ensure monthly cleaning and maintenance of all point of care testing analyzers.
  • Assists in performing Lead Nurse responsibilities in the absence of the Lead Nurse, under the supervision of an RN or provider.
  • Assists the Clinical Training Coordinator and Lead Nurse with yearly nursing staff evaluations and periodic evaluations on an as-needed basis.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the CORNERSTONE Policy and Procedure Manual and Employee Handbook.
  • Attend and participate in monthly department, All Staff and other required meetings as well as staff huddles when needed/assigned by Lead Nurse.
  • Maintain confidentiality of all aspects of CORNERSTONE including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other duties as assigned.
