About The Position
Haven Home Health & Hospice is seeking a compassionate and organized Clinical Coordinator (LPN) to join our team. The Clinical Coordinator serves as a vital link between patients, caregivers, physicians, and clinical staff to ensure quality patient care and effective communication.
Requirements
- Current Oklahoma LPN license required
- Strong communication and organizational skills
- Ability to multitask in a fast-paced environment
- Proficient computer and documentation skills
- Valid driver's license, reliable transportation, and current auto insurance
Nice To Haves
- Home Health experience preferred
- Previous clinical coordination experience preferred
Responsibilities
- Coordinate patient care and communication between physicians, clinicians, patients, and caregivers
- Process physician orders and patient care updates
- Assist with patient admissions and ongoing care coordination
- Monitor and communicate changes in patient condition
- Maintain accurate patient records and documentation
- Support clinicians with scheduling, orders, and care planning
- Participate in interdisciplinary team communication
- Assist with orientation and training of new staff
- Participate in on-call, weekend, and holiday rotations as assigned
