About The Position

Saint Luke's is seeking a compassionate and dedicated Clinical Intake Coordinator-LPN to join their supportive environment. This hybrid role involves facilitating the Home Health & Hospice referral process by receiving referrals, determining the need for additional information, communicating acceptance or decline of referrals, and processing accepted referrals. The coordinator will also prioritize and triage hospice referrals and ensure regulatory compliance during the initial screening and intake process. Employees are required to work on-site 3-4 days per week, with the option to work remotely 1-2 days per week after orientation. Remote days are scheduled on a rotating basis to ensure adequate office coverage, and work arrangements are subject to change based on business needs.

Requirements

Missouri and Kansas LPN License Required.
2 years Applicable Experience
Family Care Safety Registry - Missouri Department of Health & Senior Services
Licensed Practical Nurse (KS) - National Council of State Boards of Nursing (NURSYS)
Licensed Practical Nurse (MO) - National Council of State Boards of Nursing (NURSYS)

Responsibilities

Facilitates the Home Health & Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System.
Determines need for additional information from referral sources.
Communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.
Prioritize and triage hospice referrals.
Ensure regulatory compliance is met during initial screening and intake process.
