Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home. Reporting to the Assistant Director, Living in Place, the Clinical Coordinator, Living in Place, provides clinical support to Queens based, Aging Services programs at Breaking Ground using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community, and provides information and referral services; benefits application and navigation assistance; resource development support; and community integration programming. The case management for Aging Services is typically a 3–6 month engagement and on a referral basis. The Clinical Coordinator maintains a caseload of 30-40 clients and serves as a member of the Tenant 360 management team.
